The world may never know of the exact cause of Joan Rivers' death because her daughter Melissa never ordered an autopsy.

Sources close to NYC Medical Examiner can reveal that the TV host's only daughter was downcast after her mother's death, telling doctors not to conduct any autopsy on the TV host's body.  New York law stipulates that doctors are bound to honor a family's request not to autopsy a body if no evidence of criminality is associated with the death.

The Medical Examiner has concluded that Joan died because of oxygen deprivation leading to irreparable brain damage. Furthermore, oxygen deprivation may have been even caused by the procedure itself and not Propofol.

TMZ writes that the only way for Joan's family to find out the actual cause of the actress' death if a doctor or nurse admits a mistake.

Rivers death was said to have been claimed by 'predictable' complication brought about by the throat procedure, a report from the New York Medical Examiner can reveal. A further autopsy could actually specify what happened during the surgery, such as a puncture in the throat or other traumas.

Rivers underwent a throat procedure at the Yorkville Endoscopy. It has been since declared substandard in four major categories necessary to pass the accreditation by the New York State Health Department. The facility has until January 7 to comply with these deficiencies.

These categories are: governing body and management, surgical services, medical staff and patient rights.

In an interview with MailOnline, Dr. Jerome Waye, a Manhattan gastroenterologist who spoke on behalf of the physicians on the Yorkville staff about the investigation said that failure to pass the New York state's accreditation "is a big, big deal."

"They won't be able to take Medicare patients, and that's got to be at least half their clientele," he explained.
